This would need to be returned to Fairfax Academy with both your child’s I.D and proof of address:
Proof of ID
Either your child’s passport or birth certificate
Proof of address
Either a utility bill, bank statement or council tax bill – less than 6 months old
(Please note that tenancy agreements are not accepted as proof of address)
